How to make an appointment:
Please call Family Counseling of Columbus at the number shown above and request an appointment. Our Intake Specialist will talk with you to gather some brief information about the nature of counseling you are interested in, explain our options, and help decide how to best meet your needs. She will also ask for some basic information to help us determine an estimate for the fee that will you will be charged, whether you have insurance, and what hours or days of the week for an appointment time will work best with your schedule. Your first appointment will be a two hour intake.
When you arrive for you appointment, you will meet with the Intake Specialist to establish your fee. Once this administrative intake is completed, you will then meet your counselor.
What you need to bring with you:
- A photo ID
- Proof of income (pay stub or voucher, tax paperwork, verification of unemployment from Department labor, etc)
- Any documents from referral sources or other parties involved (i.e.: court referral, DHR referral, etc.)
- FOR MINOR CLIENTS – if you are calling to arrange counseling for a minor child, in situations where there is any sort of guardianship, custody agreements, parental rights limitations, etc., please bring pertinent documents that clarify the legal circumstances.
